This is to reiterate the enrollment and payment procedure provided in the FDA Circular 2021-007, entitled Guidelines on the Attendance/Enrollment to FDA Academy Training/Seminar Programs Offered through Online Video Conferencing Platform.
Stakeholders who are interested to enroll in any of the FDA Academy training/seminar programs being offered with a corresponding registration fee shall register through the provided link and wait for the email confirmation and Course Assessment Slip (CAS) to be sent by the FDA Academy. Only the registered participants who received the CAS shall proceed to payment following the instructions provided by the FDA Academy.
Payments made without the required registration and CAS shall automatically be forfeited in favor of the government and shall be non-refundable/non-transferable.
Further, payment shall be made solely through the FDA Academy’s bank account with the following details:
Bank : Development Bank of the Philippines (DBP)
Account Name : FDA ACADEMY TRUST FUND
Account Number : 00-0-00291-430-9
The FDA shall not be held liable for any payment made not in accordance with the said Circular.
